6. Include your name, mailing address, telephone number, and email address

Meta VPS.org tirċievi Avviż xieraq, aħna se:

• Immediately remove or disable access to the allegedly infringing material
• Terminate the accounts associated with the material (if applicable) in accordance with the DMCA
• Notify the affected user of the takedown

Jekk temmen li l-kontenut tiegħek tneħħa bi żball jew li għandek l-awtorizzazzjoni biex tuża l-materjal, tista' tissottometti kontro-notifika lill-Aġent tad-DMCA tagħna bl-informazzjoni li ġejja:

• Your physical or electronic signature
• Identification of the material that was removed and its location before removal
• A statement under penalty of perjury that you have a good faith belief that the material was removed as a result of mistake or misidentification
• Your name, address, telephone number, and a statement that you consent to the jurisdiction of the federal court in your district
